1. Skin Ligthening Skin Care Regimen: This applies to almost everyone. In some form or another, the skin needs to be on ingredients that will help reduce pigment. An effective program may replace the need for any further procedures because the right regimen, which always includes some form of retinoid and meticulous sun protection plus or minus hydroquinone, can give dramatic results. Factors to consider are a person's willingness to adhere to a skin care routine, tolerance for possible redness and peeling and patience with a more gradual process. Even patients who at first are deterred by the notion of following several steps each morning and night often come around when they see just how effective and actually enjoyable these programs can be. This is the one "pain-free" option.
2. Chemical Peels: There are a range of chemical peels which vary in intensity and effectiveness. The stronger peels need to be done less frequently but give more redness, peeling and even mild discomfort. Weaker peels are more pleasant but less effective and need to be repeated regularly to see improvement. Peels are safer and more effective when performed on patients who follow a skincare regimen that prepares and protects their skin. I use gentle, frequent peels for patients who prefer not to have to follow multiple steps with their skin care at home. These patients prefer to have consolidated days of peeling rather than the more protracted peeling that can be seen when adjusting to an aggressive skin care regimen at home. Chemical peels are an excellent option if a person is looking to lighten their brown spots AND has an acne tendency. The peel will simultaneously treat both. It's also helpful when crepiness around the eyes is a concern because certain peels can be performed on the eyelids in addition to the face. Melasma patients also do very well with chemical peels as long as they are on the best pre and post treatment skin care. Prices per peel range from $100-$360 in my office and generally includes treatment of the face, neck and decolletage area simultaneously.
